JOB TITLE: Licensed Practical Nurse (LPN)


POSITION SUMMARY: The Licensed Practical Nurse (LPN) is responsible for resident nursing care under the direction of the Registered Nurse. The nursing care is based on the comprehensive care planning process. The LPN provides leadership for the nursing assistants and all staff when in charge of the building. The LPN observes, documents and reports findings to the Registered Nurse.

QUALIFICATIONS: Must have current registration, as a Licensed Practical Nurse, in the State of Wisconsin. Must have successfully completed an approved School of Nursing. Must possess the ability to communicate verbally and in writing when directing the care of residents. Must be able to perform essential functions of the position with or without reasonable accommodation.

Must qualify for employment, after criminal background study, per guidelines of Wisconsin Department of Human Services.

RESPONSIBLE TO (SUPERVISOR): Registered Nurse, Nurse Manager and Director of Nursing

SUPERVISES (IF APPLICABLE): CNAs and all staff when in charge of the building

WORKING HOURS: Work hours per week as assigned

RESPONSIBILITY: To Facilitate the Comprehensive Care Planning Process

Systematically collects data about residents through interview of resident and family, observation, review of medical records.

Applies data gathered, plus knowledge to facilitate care planning process.

Assists in the development of care plan with input from the interdisciplinary team.

Documents changes in the care plan under the direction of the RN.

Communicates necessary information from the care plan to primary caregivers.

RESPONSIBILITY: To Provide Resident Care in Compliance with Established Nursing Policies and Procedures According to State and Federal Regulations and Nurse Practice Act

Is knowledgeable of and abides by the importance of the residents Bill of Rights.

Uses standards of care and specific policies and procedures to perform nursing care duties for residents.

Allows autonomous decision making to resident whenever possible.

Abides by and promotes the Continuous Quality Improvement program of the Rehabilitation Center and completes audits as designated.

Is involved in making necessary recommendations to nursing administration that directly and indirectly affect care given to residents.

RESPONSIBILITY: To Ensure Safe Environment for Residents and Staff and to Protect Self and Others From Blood Borne Pathogens

Correctly uses transfer belts for transferring and lifting residents.

Correctly uses Hoyer or mechanical lift following the proper procedure as designated by Monarch Healthcare Management.

Appropriately and correctly applies and releases restraints.

Knows and follows the Standard Precautions Policies.

Appropriately wears protective equipment required for standard precautions (i.e., gloves, face shield, fluid resistant gown).

Prevents the spread of disease/infection by following proper infection control procedures, to include proper handwashing and disinfection/cleaning schedules.

Follow procedure to bag infectious linens and other infectious equipment, and disposes of them properly.

Maintains safe clean environment in resident's rooms, and facility including wiping up spills immediately.

Understands the vitally important task to answer any and all call lights promptly and courteously.

Uses correct body mechanics at all times.

Follows safety policies and procedures as outlined by Monarch Healthcare Management.

RESPONSIBILITY: Provides Professional Nursing Cares to all Residents and Exemplifies Leadership on the Nursing Unit

Receives report from nurse making written notes as necessary.

Articulates expectations and delegates appropriate job duties to co-workers.

Follows up on directions given to co-workers and assures completion of duties.

Assesses knowledge and skill of supportive nursing personnel to assure safe practice of nursing staff and participates in the evaluation process.

Confronts, corrects and explains unprofessional behavior to staff and completes counseling forms and written warnings.

Prioritizes work assignments consistent with the number and levels of staff available that shift and the acuity of residents care.

Knows and is able to implement and coordinate fire, tornado and disaster procedures.

Take necessary steps to replace appropriate nursing staff to maintain proper staffing level. This may include working as the staff nurse.

RESPONSIBILITY: Has Knowledge and Consistently Follows Standards of Documentation and Written Communication

Completes form on any incidents or errors involving residents, staff, visitors or self, and communicates to the Nurse Manager.

Completes charting in accordance with health information standards and facility policy.

Data is systematically collected, is communicated through report and documented in chart.

Gathers and documents data on admissions and discharges.

Facilitate all physician orders and completes procedures to assure accurate follow-up.

Initiates, reports and documents nursing orders.

Provides report to CNA's and other professional staff, verbally or written about daily resident care needs and/or significant changes in condition.

RESPONSIBILITY: Complies with Pharmaceutical Policies of the Rehabilitation Center

Administers medications accurately according to policy, including handwashing and 5 rights.

Has minimal medication errors.

Signs out medications and treatments appropriately.

Completely and accurately obtains and transcribes Physician orders for medication and treatments.

Uses proper procedure for label changes.

Follows proper emergency kit procedures.

Follows control substance policy, including count at the end of each shift.

Documents PRN's according to policy.

RESPONSIBILITY: Promotes and Establishes Healthy Relationships and Uses Professional Conduct with Supervisors, Other Health Care Professionals, Residents, Families and Visitors

Arrives at work area on time and ready to begin work. Is able to prioritize and be organized and efficient when interacting and communication.

Follows dress code, wears name tag, and limits personal phone calls.

Maintains resident confidentiality at all times. Understands the need for complete privacy of all health care data.

Follows policy on absenteeism, tardiness and call-ins as outlined in Policy Manual.

Is courteous and cooperative with residents, families, visitors, supervisors and co-workers.

Promotes a positive and professional image both in the facility and in the community.

RESPONSIBILITY: Maintain Current Levels of Education and Facilitate the Implementation of New Policies and Procedures with the Facility

Provides Rehabilitation Center with current license and keeps updated.

Attends continuing education/in-services pertinent to your department and designated by the ADON.

Attends nurses meetings at the Rehabilitation Center.

Attends all staff education of the Rehabilitation Center relating to safety, infection control and disease process.

Attends educational programs on Continuous Quality Improvement, customer satisfaction and other such programs. Participates in inspections and audits as designated.

Maintains current knowledge of Wisconsin Department of Health regulations.

Maintains and applies knowledge of current policies and procedures
