JOB DESCRIPTION AND RESPONSIBILITIES


PERFORMANCE STANDARDS/EVALUATIONS



JOB TITLE: Registered Nurse (RN)

DEPARTMENT: Nursing


POSITION SUMMARY: The Registered Nurse is responsible for implementation and follow through of the nursing process. This includes systematic data collection, assessment, development of care plan, implementation of care plan and revision of plan. The Registered Nurse manages and supervises the care of residents through a well defined care delivery system. He/She also supervises other nursing personnel to assure quality outcome of care.


QUALIFICATIONS: Must have current registration, as a Registered Nurse, in the State of Wisconsin. Must have successfully completed an approved School of Nursing. Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider. Must possess the ability to communicate verbally and in writing when directing the care of residents. Must be able to perform essential functions of the position with or without reasonable accommodation. Must qualify for employment, after criminal background study, per guidelines of Wisconsin Department of Human Services.


RESPONSIBLE TO (SUPERVISOR): Nurse Manager, Directors of Nursing


SUPERVISES (IF APPLICABLE): CNAs, all staff when Administrator and Department Directors are not present.


WORKING HOURS: Dayshift 6am-230pm or PM Shift 2pm-1030pm or 12 hr shift 6am-630pm

RESPONSIBILITY: To Facilitate the Comprehensive Care Planning Process and Apply Clinical Standards of Nursing Practice


Systematically collects data about residents through interview of resident and family, observation, review of medical records.


Assesses residents and applies knowledge to facilitate care planning. Applies nursing diagnosis as pertinent.


Assists in the development of care plan with input from the interdisciplinary team.


Implements care plan by assigning responsibility and communicate residents' needs to designated staff.


Assessing the effectiveness of the resident plan of care ongoing and documenting goals and outcomes met or changed.


Provides clear and accurate information and documentation of the above, continually.


RESPONSIBILITY: Has Knowledge of and Consistently Applies Established Nursing Policies and Procedures According to State and Federal Regulations and Nurse Practice Act


Is knowledgeable of and abides by the importance of the residents' Bill of Rights.


Uses standards of care and specific policies to perform nursing care duties for residents.


Allows autonomous decision making to resident whenever possible.


Abides by and promotes the Continuous Quality Improvement program of the Rehabilitation Center and completes audits as designated.


Is involved in making necessary recommendations and changes to nursing administration that directly and indirectly affect care given to residents.


Follows all nursing procedures to insure standardized care and quality treatment of each resident.


RESPONSIBILITY: To Ensure Safe Environment for Residents and Staff and to Protect Self and Others From Blood Borne Pathogens


Follows safety policies and procedures

Knows and follows OSHA's guidelines for blood borne pathogens.


Appropriately and correctly applies and releases restraints.


Knows and follows the Standard Precaution Policies.


Prevents the spread of disease/infection by following proper infection control procedures, to include proper handwashing and disinfection/cleaning schedules.


Follows procedure to appropriately bag infectious linens and other infectious equipment and disposes of them.


Maintains safe clean environment in residents' rooms and facility including wiping up spills immediately.


Assures and participates in answering any and all call lights promptly and courteously.


Uses correct body mechanics at all times.


RESPONSIBILITY: Supervises and Manages LPN's and CNA's, Manages all Staff When in Charge of Building


Articulates expectations and delegates appropriate job duties to co-workers.


Follows up on directives given to co-workers and assures completion of duties by receiving report from departing nursing staff.


Assesses knowledge and skill of supportive nursing personnel to assure safe practice of nursing staff.


Confronts, corrects and explains unprofessional behavior to these staff.



Prioritizes work assignments consistent with the number and levels of staff available that shift and acuity of residents' care.


Knows and is able to implement and coordinate fire, tornado and disaster procedures.


Take necessary steps to replace appropriate nursing staff to maintain proper staffing level. This may include working as the staff nurse.


RESPONSIBILITY: Has Knowledge and Consistently follows Standards of Documentation and Written Communication


Completes charting in accordance with health information standards and facility policy.


Data is systematically collected, is communicated through report and documented in medical record.


Gathers and documents data on admissions and discharges.


Facilitate all physician orders and completes procedures to assure accurate follow-up.


Initiates, reports and documents nursing orders.


Provides report to CNA's and other professional staff, verbally or written about daily resident care needs and/or significant changes in condition.


RESPONSIBILITY: Complies with Pharmaceutical Policies of the Rehabilitation Center


Administers medications accurately according to policy (including handwashing and 5 rights).


Has minimal medication errors, and follows company policies for reporting and documenting such.


Signs out medications and treatments appropriately.


Completely and accurately obtains and transcribes Physician Orders for medication and treatments.


Uses Proper procedure for label changes.


Follows proper emergency kit procedures.


Follows control substance policy to include count at the end of each shift.


Documents PRN's according to policy.


RESPONSIBILITY: Promotes and Establishes Healthy Relationships and Uses Professional Conduct with Supervisors, Other Health Care Professionals, Residents, Families, Visitors, and Co-Workers


Arrives at work area on time and ready to begin work. Is able to prioritize and be organized and efficient when interacting and communicating.


Follows dress code, wears nametag, and limits personal phone calls.


Maintains resident confidentiality at all times. Understands need for complete privacy of all health care data.


Is courteous and cooperative with residents, families, visitors, supervisors and co-workers.


Follows policy on absenteeism, tardiness, and call-ins as outlined in Personnel Policy Manual.


Promotes a positive and professional image both in the facility and in the community.



RESPONSIBILITY: Maintain Current Levels of Education and Facilitate the Implementation of New Policies and Procedures With the Facility


Provides Rehabilitation Center with current license and keeps personnel file updated.


Attends continuing education/in-services pertinent to your department and designated by the ADON.


Attends nurses meetings of the Rehabilitation Center.


Attends all staff education of the Rehabilitation Center relating to safety, infection control and disease process.


Attends educational programs on Continuous Quality Improvement, customer satisfaction and other such programs.


Maintains current knowledge of Wisconsin Department of Health regulations.


Maintains and applies knowledge of current policies and procedures as outlined


***** Sign on bonus will be given to employees hired during the time of advertisement. Bonus will be distributed at 6mo anniversary and 1 year anniversary considering employee stay in compliance with rules set and given at time of hire.
