We are looking for enthusiastic Family Practice Nurse Practitioners to join our team, providing vital health evaluations in Dane and Rock counties, Wisconsin. This is your chance to deliver meaningful healthcare services in a flexible, supportive environment with a competitive hourly rate. In this role, you will conduct 5-6 in-home health assessments daily, performing annual risk assessments and chronic condition reviews. This position is assessment-only—no treatment or prescribing involved—making it ideal for providers interested in preventive care and patient education. You will be responsible for identifying urgent health issues and escalating them to the patient's primary care provider. Approximately 70% of the work involves telehealth consultations, with the remaining 30% in-home visits, primarily scheduled for providers residing in the counties. This is a full-time (40 hours/week) or part-time (20 hours/week) role with flexible scheduling, typically Monday through Friday, 8 am to 5 pm, including a 1-hour lunch break. Schedules are released 2-3 weeks in advance, allowing you to plan your workload efficiently. The position offers paid for full days regardless of patient volume, provided availability is entered into Salesforce. Orientation and ongoing education are prioritized, with 1-3 hours of paid orientation and additional education modules completed during work hours. Candidates must have an active Wisconsin Medical License, be enrolled and credentialed in Medicard, and possess excellent communication and organizational skills. Prior experience in home health assessments or telehealth is a plus. Travel is required; mileage will be reimbursed according to IRS standards.
