Job Description:
Ascension is seeking a dedicated and compassionate RN-Case
Manager to join our faith-based healthcare organization, where
innovation meets personalized care. In this role, you will be a
pivotal part of our team, coordinating and optimizing healthcare
plans across our network of hospitals, clinics, and healthcare
providers. You will be instrumental in facilitating patient care,
ensuring the efficient delivery of healthcare services, and
supporting patients through their healthcare journey by developing
tailored care plans based on individual needs. Assess new patients,
reviewing their medical history and any referral notes necessary
for treatment planning. Develop, implement, and evaluate
individualized patient care plans in collaboration with other
healthcare professionals. Regularly review and update care plans to
fit patient needs and responses. Serve as a primary liaison between
patients, their families, and healthcare providers to ensure
cohesive and personalized health management. Advocate for patient
needs and ensure that all aspects of care are coordinated,
including medical treatment, physical and psychological support
services. Educate patients and their families on health maintenance
and disease prevention. Maintain comprehensive documentation of
patient care and progress. Participate in the training of new staff
and provide ongoing guidance to junior nurses. Strong interpersonal
and communication skills Expertise in creating and managing
comprehensive healthcare plans Ability to work collaboratively
across multiple healthcare disciplines Proficient in healthcare
management software and tools Strong organizational skills and the
ability to handle multiple cases simultaneously Medical Insurance

Registered Nurse (RN) license in the state of practice
Certification in Case Management (CCM) preferred but not
required
