shifts scheduled Monday through Friday. Potential weekends in the
future depending on program growth. This position will work
primarily in patient homes but has a physical work location at the
Administrative Services Building in Madison, WI. Hours may vary
based on the operational needs of the department. /n Pay: /n /n -
This position may be eligible for a $2,000 sign-on bonus (pro-rated
based on FTE). /n - Additional components of compensation may
include: /n /n - Evening and night shift differential /n - Weekend
differential (if implemented) /n - Mileage reimbursement /n /n /n

(OT) to join our Home-Based Hospital Care Team! You will: /n /n -
Deliver acute, hospital-level care to patients in their homes using
an evidence-based and patient-centered therapy approach. /n - Help
patients regain independence, improve daily function, and safely
navigate their home setting, reducing hospital stays and improving
outcomes. /n - Work in collaboration with the interdisciplinary
team to ensure that patients receive exemplary care in their home.

Therapy Required /n /n /n /n Work Experience/n /n /n /n - 2 years

in the State of Wisconsin Upon Hire Required /n - Basic Life
