Job Title: Home Health Physical TherapistVNA Health Group is seeking a skilled Physical Therapist to provide high-quality care to patients in their homes.About the Role:

• Assess and evaluate patients for physical therapy needs and develop personalized plans of action in collaboration with physicians.
• Provide skilled physical therapy care to patients in their homes, promoting independence and improving overall well-being.
• Document patient progress and update care plans accordingly.
• Identify counseling needs and plan appropriate teaching strategies to support patients and their families.
• Monitor quality and appropriateness of services provided and adhere to agency infection control and safety policies.Requirements:
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and effective communication and interpersonal skills.
  • Valid NJ driver's license.Benefits:
