We're accomplishing this first through:High-quality medication-assisted treatment (MAT) led by physicians and licensed medical providers in a convenient outpatient environment.Hand-on care, providing our patients with psychosocial support and intensive care coordination with a wide network of partners in every community.Individualized mental health support, through one-on-one counseling, peer support or group sessions.CleanSlate has treated more than 100,000 patients in over 75 centers across the country, working closely with our partners to save and change lives.Our expert care brings hope and help to patients so they can achieve the lives they want and deserve.Responsibilities:As a Medical Assistant II, you will provide direct patient care, support for medical providers, and administrative support for other clinic functions to ensure our patients receive the highest quality care while maintaining compliance with the established policies and procedures of CleanSlate.Prepares patients for their health care visit with a focus towards excellent patient carePerforms phlebotomyReview and/or update patient medical historyObtain Vital Signs and COWS score and other assessments as neededPerforms medication injectionsSchedules new and recurring patient appointments for Nurse Practitioners, Physician Assistants, Counselors and physicians as neededScanning of patient documents into Electronic Health RecordPerform medication countsContact patients via phone as required for general follow up, prescription management, communicate lab results and to schedule appointmentsMaintain laboratory supplies & PPEMaintain medication storage onsite by maintaining storage logs and equipment as requiredAssist providers with prescription management by communicating with patients and pharmaciesCollect urine screen and other laboratory tests including CLIA waived testsSample prep for send-out testingMaintain all logs and required checks (i.e. Temperatures logs, expired/dispose of medications, etc.).Maintains patient confidentiality and protects operations by keeping patient care information confidentialAll other tasks or projects, as assignedQualifications:This position is subject to a background check for any convictions directly related to its duties and responsibilities. We will consider for employment all qualified applicants with arrest and conviction records and will assess the details of any criminal conviction in a manner consistent with applicable law.
