This position acts as an advocate by being a primary contact for Prevea patients, Prevea 360 members and potential patients who need support and/or guidance when navigating Prevea Health and helps connect them to the appropriate entity in which they seek for both Prevea Health and Prevea 360.
Further, this position may assist with review and management of care gap closure through patient outreach and assisting with appointment scheduling and care coordination.
Essential Functions (primary responsibilities of the position in order of importance)
Assist patient with the navigation of the Prevea Health care system (includes, but not limited to establishment with PCP, Specialty, myPrevea, Prevea.com, appointment scheduling and care coordination)
Onsite visits to meet with brokers, HR representatives and others to discuss how the Advocate can support their employees/members
Connecting patients with information obtained from customer support within Prevea 360
Frequent communication with the Prevea business and strategic growth teams on workflows, goals, strategy
Frequent communication with the Prevea 360 business team to ensure smooth transition for newly enrolled members
Review open care gap reports issued, outreach to member, facilitate the scheduling of a patient appointment if able, and update file

Complies with all policies and standards of position, department, and organization.
Complies with HIPAA for the protection of patient and confidential information
Diploma or GED
High school graduation or its equivalent
Certification from a Medical Office Specialist, Medical Assistant, Patient/Customer service representative or other medical clerical program
Experience within healthcare setting
Experience working with health insurance
Experience working in customer relations
Licenses and Certifications
Certified, Registered, or Certified Clinical Medical Assistant
Knowledge, Skills and Abilities

Ability to form work/patient relationships with patients and peers
Ability to prioritize work and meet changing demands and stress deadlines.
Fosters teamwork through effective written and verbal communication with all Prevea colleagues.
