Summary The Process Improvement Facilitator leads improvement
projects that help teams across Prevea Health work more efficiently
and deliver better experiences for patients and staff. This role
works directly with departments to identify issues, map out
solutions, and support successful implementation. In addition to
managing projects, the Facilitator builds internal skills by
coaching teams and providing resources to support their own local
improvement work. This position plays a key role in translating
big-picture strategies into practical changes that make a
day-to-day difference. What you will do - Process Improvement
Strategy & Execution Under the direction of the Medical Director of
Process Improvement, leads organization-wide, clinical and
administrative process improvement initiatives by identifying
inefficiencies, analyzing data, and applying Six Sigma
methodologies. Works with leadership and frontline teams to develop
and implement solutions that enhance patient care, reduce costs,
and streamline workflows. Supports interdisciplinary teams in
improving existing workflows or designing new operational
processes. Collaborates with teams to ensure data-driven
decision-making and sustainable improvements in patient care and
service delivery. Partners with department supervisors and managers
to review patient satisfaction data and create targeted action
plans for improvement. - Training & Capacity Building Develops and
delivers educational programs to build internal expertise and
capabilities in process improvement. Coaches staff and leaders on
problem-solving techniques, Six Sigma principles, and data-driven
decision-making to empower teams to manage their own improvement
initiatives. Serves as a consultant to departments and clinics as
they implement their own process improvement efforts. - Strategic
Project Alignment & Oversight Manages the intake, evaluation, and
prioritization of process improvement projects to ensure they align
with Prevea Health’s strategic goals. Establishes structured
workflows to evaluate feasibility, resource availability, and
potential impact. Maintains a centralized portfolio of projects
(proposed, active, completed), ensuring transparency,
prioritization, and strategic alignment. Provides regular status
updates and recommendations to leadership to support informed
decision-making. - Performance Measurement & Sustainability Defines
key performance indicators and measurement strategies to assess the
impact of process improvement initiatives. Develops dashboards,
reporting tools, and visual management systems to monitor project
progress and inform decision-making. Tracks business value
realization, ensuring that completed projects achieve expected
efficiencies and cost savings. Applies root cause analysis (RCA)
and failure mode effects analysis (FMEA) to refine processes and
ensure long-term sustainability. Works with operational leaders to
standardize and integrate successful improvements into ongoing
workflows. - Stakeholder Engagement & Change Adoption Partners with
the Manager of Change Management to ensure that improvement
initiatives are successfully adopted and sustained. Engages
leaders, frontline staff, and key departments to drive
participation, address resistance, and reinforce a culture of
continuous improvement. Develops communication strategies to
maintain transparency and engagement throughout project lifecycles,
ensuring alignment between leadership and operational teams.
Education Qualifications - Bachelor's Degree in Project Management,
Healthcare Administration, or related field Required - Master's
Degree in Project Management, Healthcare Administration, or related
field Preferred Experience Qualifications - 1-3 years Experience in
project management, process or continuous improvement, operations,
or workflow optimization in healthcare or similarly complex,
regulated environments. Required - 1-3 years Experience working
with healthcare professionals in various levels of an organization.
Preferred Skills and Abilities - Demonstrated problem solving and
leadership skills. - Experience with interactive presentations to a
group. - Demonstrated professional experience working on strategic
projects with multiple priorities. - Demonstrated ability to
coordinate deliverables and dependencies across business units and
communicate and collaborate with all levels, including executives.
- Strong written and verbal communication skills with the ability
to clearly communicate complex information. - Experience and
demonstrated ability to teach new skills - Ability to build and
maintain effective business relationships with key organizational
stakeholders, business partners and senior leaders. - Proficiency
with the Microsoft Office Suite. - Demonstrated experience applying
structured improvement methodologies to drive measurable outcomes.
- Ability to collect, analyze, and interpret operational data to
identify process inefficiencies and guide evidence-based
decision-making. - Familiarity with process mapping, root cause
analysis, and visual management tools to support sustainable
workflow redesign. Licenses and Certifications - Project Management
Professional (PMP) Certification awarded by the project Management
Institute (PMI), Certified Associate in Project Management (CAPM),
Six Sigma Black Belt Certification or other recognized project
management certification or advanced learning degree in Healthcare
Management, such as Master’s Certification in Project Management or
